CodeXcellence, Inc., will provide education on combination visits which includes, documenting preventative visits done in conjunction with an evaluation and management visit and evaluation and management done the same day with a procedure. Also, the required documentation for an initial or subsequent visit that results in a decision for surgery during an inpatient or outpatient visit with proper modifier use.
